UPDATED FORM: EVENT PLANNING AND APPROVAL FORM

The updated Event Planning Approval form document has been posted in the "Forms" box to the left under the title Event Planning Form.
Updates to the form include:
- New question regarding whether it's a Mosaic event or not
- Addition of Classified Staff signature line
- Addition of ESS Admin Manager Check In Meeting request
- New 2017/18 indirect rate
- Additional line to indicate UNPAID registrations (such as staff, comped attendees, etc).
