Delta Dental Enrollment

SCOE is offering a special, one-time only open enrollment period for Delta Dental for the month of October 2017. During this period, employees will be able to make changes to their Delta Dental coverage such as adding dependents.

To make changes to your Dental Dental coverage, please follow these steps:

1. Print the Delta Dental Form.
2. In section A, enter your personal information. If there is information that you don't know such as "Date Employed", leave it blank and HR will input that information for you.
3. In section C, list all covered dependents including dependents being added with their social security numbers and dates of birth.
4. In section D, sign and date the form.
5. Return completed form to Diane or Marlene in Human Resources by October 31st by 4:30 p.m.
