Open Enrollment for Voluntary Supplemental Group Life Insurance
The annual open enrollment period to enroll in or change voluntary supplemental term life insurance begins May 1 and ends June 30, 2023.
If you wish to enroll or increase your Voluntary Term Life coverage, please:
- Complete the Insurance Enrollment Form and return to Diane Perkiss in the HR department no later than June 30, 2023.
- You may enroll or increase your coverage to the maximum “Guaranteed Amount” without having to complete an Evidence of Insurability Form and will not have to go through medical underwriting.
- Please fill out the "Employee" section beginning with your Name, on. HR will complete the "Employer" section and the coverage amounts section for the "Employee Amount" and "Spouse* Amount".
- This form may not be sent through email, due to containing employee’s sensitive personal information – IT WILL BE BLOCKED BY SCOE’s EMAIL SECURITY SYSTEM.
- Complete the Evidence of Insurability Form (if applicable)
Any enrollment over the “Guaranteed Amount” WILL require the Evidence of Insurability completed and submitted with your Enrollment form to Diane Perkiss in the HR department, no later than June 30, 2023 - This form may not be sent through email, due to containing employee’s sensitive personal information – IT WILL BE BLOCKED BY SCOE’s EMAIL SECURITY SYSTEM.
